Anal Botox (HoleTox)

Anal Botox -- known as HoleTox -- involves the injection of botulinum toxin into the anal area. Its best-established use is therapeutic: for chronic anal fissure it relaxes the internal sphincter, and colorectal surgery guidelines place it alongside topical therapy as a first-line option, with healing in roughly two thirds of patients and meaningful pain relief in most. It is also requested for cosmetic smoothing of the perianal area, which is an off-label use with no published efficacy data, so we will not promise you a cosmetic result.

Straight answers

Around 2 to 3 months, and the effect is temporary by design. For fissures, recurrence is common -- up to about 40 to 50 percent within the first year -- and repeat treatment works about as well as the first. You should also know the main risk: temporary loss of control over gas affects up to about 18 percent of patients and temporary loss of stool control about 5 percent. Both typically resolve as the toxin wears off, and lower doses appear to reduce the risk.
